Executive Summary 
 
The accidental release prevention and emergency response policies: This facility complies with NFPA-58 requirements for LP-Gas storage, and it is our policy to adhere to all applicable federal, state, and local laws. If an emergency were to occur, it is our policy to notify the Kingman Fire Department requesting their response. 
 
A description of our facility and regulated substance handled: Equatorial Mineral Park, Inc. (EMPI) is an in-situ leach, copper cathode producing mine. The copper cathode is then shipped to a refinery. EMPI has two separate propane tanks considered covered processes and are listed as follows: one 3,000 gal. tank used for space heating purposes, and one 6,000 gal. tank used to supply LPG for a Raypak hot water boiler. These tanks are rented from the Ferrelgas Company located in Kingman, AZ. Ferrelgas also services and maintains the tanks. 
 
The worst-case release scenario: Our worst-case scenario would be the failure of our 6,000 gal. storage  
tank when filled to 88% at 60F0. According to EPA's overpressure reference table, the worst-case distance to a 1 psi overpressure event is 0.2  miles. Since EMPI is remotely located in an unoccupied mountainous area, the worst-case scenario will not affect anyone beyond our property. 
 
The general accidental release prevention program: EMPI  conforms to the federal Emergency Planning and Community Right-to-know-Act as well as the state statutes for the development and submission of  a site specific Emergency Response Plan. We have submitted this plan to the local fire department as well as to the state authorities. Our propane systems are designed, installed, and maintained in accordance with NFPA-58 and state law. 
 
Five-year accident history: EMPI has never had an accident event involving our propane tanks much less an event that caused deaths, injuries, property or environmental damage, evacuations, or shelterings in place. 
 
The emergency response plan: In the event of an emergency in 
volving our propane systems, it is our policy to evacuate personnel to a pre-designated safe refuge point then initiate our emergency notification procedure. We will notify the Kingman Fire Department requesting their response to this emergency. 
 
Planned changes to improve safety: EMPI currently trains our employees annually on all aspects of our Emergency Response Plan. All of our tanks have collision barriers, bold flammable warning signs, "No Smoking within 50' " signs, and the NFPA diamond system. Tank inspections are completed monthly through our Spill Prevention, Control and Countermeasures plan.
