Executive Summary SUPERVALU Champaign Distribution Center is committed to providing a safe and healthful work environment for its employees and visitors. It is important to establish emergency action plans to protect life and property. Experience has shown that actions taken during an emergency are seldom effective unless planned in advance. For example, a fire or other emergency could require the rapid evacuation of large numbers of employees from the facility. It is imperative that plans to accomplish evacuation quickly, efficiently and safely have been developed and communicated to all employees. SUPERVALU Champaign Distribution Center is a grocery distribution facility, servicing over 300 retail stores in a five-state area. Because we store and ship perishable and frozen food products, our refrigeration system is operated using 30,000 lbs. of Anhydrous Ammonia. The worst case scenario would be if the largest vessel ruptured, releasing 4020 lbs. into the atmosphere. This release would effect a 1.14 mile radius with a population of 728 residents. The ammonia system piping has been completely guarded to reduce the risk of accidental damage from equipment. Any expansion made to the facility, the piping system is installed outside above the roof to eliminate exposure. The Champaign Distribution Center is covered by OSHA's PSM rule. We are dedicated to the prevention of catastrophic releases and minimizing the consequences of such release. Employee involvement, education and training are conducted on a regular basis to maintain employee understanding and awareness of PSM information. The Champaign Distribution Center has not had ANY reportable accident history in the last five years. The Emergency Response Program of the Champaign Distribution Center has been developed and coordinated with the LEPC, ESDA and the Champaign and Urbana Hazmat teams. The local Hazmat teams will conduct bi-annual drills at the facility, along with working knowledge of the ammonia system blueprints. The employees are trained in orientation and refresher training on PSM is held annually. The Distribution Center conducts four emergency evacuation drills on an annual basis. Any release that registers 200ppm will result in immediate notification to 911. The Champaign/Urbana Hazmat team will respond to the call, assisted by distribution center emergency responders. The facility maintains a "Telephone Tree", put into effect in the event of an emergency, to notify the community. An on-site medical facility is staffed by an EMT-I and will be available to assist with any injuries. The facility has a written Emergency Action Plan, which is reviewed by employees and management annually, along with the four evacuation drills. Each warehouse has their own P.A. with microphones located in the office. These systems are to be used to make announcements related to emergency situations such as evacuation notices, etc. There is a P.A. system in the administration building; however, the system is not electronically linked to the warehouse. The microphone is located at the receptionists desk in the main lobby. Audible Evacuation Alarms The fire alarm in the facility is a short, intermittent tone, aired over the P.A. system. The alarm is activated from push button stations located in the Grocery, Perishable and Transportation offices. Local alarms, such as manual pull stations, are available in the office structure only. Activation through pulling down the lever will ring bells in the office area only. Property Conservation - Sprinkler Systems/Welding All sprinkler control valves are equipped with padlocks, with the exception of buffalo boxes. Maintenance personnel are the only individuals authorized to approve t he shutdown of the sprinkler system. All sprinkler systems are checked by Maintenance on a weekly basis. Records are maintained in the Maintenance office. All hot work cutting and welding must be performed in accordance with Factory Mutual specifications and SUPERVALU policy. Permits are issued through Building Maintenance and logged. Smoke alarms are located in the office facility and conference rooms. When activated, bell alarms will sound through the office area and conference rooms. EVACUATIONS Organization The facility has developed an evacuation team that is responsible for the evacuation plan during rehearsals and actual emergencies. The team is comprised of department managers, Building Maintenance personnel and the Risk Control Manager. The Risk Control Manager directs the activity of the team with support from senior management. Team members have been issued a copy of the Emergency Action Plan and trained in all phases of the plan, including use of available commu nications, crowd control and resource utilization. Certain team members, i.e., Maintenance mechanics, have been certified in first aid and CPR to facilitate basic medical care to injured employees. Duties of the Evacuation Team The team will give priority to those areas directly involved in an emergency and adjacent areas. The most important aspect is to determine the nature of the alarm. Movement to exits will be controlled; personnel will not be allowed to run or act in any way to create panic. Area coordinators will hold head count at the assembly area. Assistant coordinators will help in the evacuation of the facility. Any missing person believed still to be in the facility must be reported to the Risk Control Manager or the designated person in charge. Employees shall not re-enter the facility without permission of the Urbana Fire Department. Advise employees to remain clear of emergency vehicles at all times. Authority to Evacuate Once the fire alarm has been sounded, t he facility must be evacuated. Prior to the alarm, evacuation is a management decision. Detection and Reporting If fire is detected or the start of the fire is witnessed, it must be reported immediately to the Supervisor. Delays in reporting fires, because of heroic but ineffective fire-fighting, results in loss of time and can allow a simple fire to spread. Practice Evacuations and Training A minimum of two building evacuation drills and two tornado drills will be held each year. Assembly Areas * Primary "A" sites: Main Office - white sign, Maintenance - Red sign, Transportation/Garage - yellow sign, Perishable - green sign, Grocery - blue sign. * Secondary "B" sites: All departments - orange sign. Evacuation Procedures In the event of an actual evacuation, employees should do the following: * Proceed immediately to the nearest exit. Follow the directions on the signs in your department. All personnel should know the location of at least two (2) exits prior to an emergency. * Stay as low to the floor as possible if smoke or vapor fills the area. * Congregate in the assembly areas. * Remain clear of approaching emergency vehicles. (For specific department evacuation plans, please review the procedures following this text.) Physically and/or Mentally Disabled Personnel The evacuation team member for each area will make provisions for the evacuation of any disabled person who needs assistance. Aisles Aisles leading to the emergency exits (those discharging to the outside) must be as wide as the emergency exits they serve. They must be kept free of obstructions to permit both evacuation of personnel and the movement of emergency equipment. Aisles may not be used for temporary storage of materials or work in progress. Exits The City of Urbana requires exits to meet the following criteria: * Emergency exits will be properly identified. * Exit door location signs will be properly illuminated. * Exits may not be blocked at any time. * Exit doors will open ea sily and swing out. * Exit doors will be self-closing. * Exit doors will remain closed when not in use. * Confusing no-exits will be clearly marked for what they are. Emergency lighting to ensure safe, quick evacuation has been installed throughout the building. Lighting will be tested at least quarterly. Emergency Stairwells The following requirements must be met. * Stairwells will be kept free of obstruction. * Stairwells will be equipped with proper handrails. * Proper illumination will be provided in the stairwells. * Stair treads and risers will be properly maintained. EMERGENCY EVACUATION PROCEDURES Department: Grocery Warehouse In the event of an evacuation, the following guidelines should be implemented to ensure employee safety. NON-SUPERVISORY PERSONNEL: * Upon hearing or receiving instructions to evacuate the building, turn off any equipment in use. * Walk to nearest exit. * Once outside, proceed to the primary assembly area, designated by a blue sign, or the seco ndary assembly area, designated by the orange sign, after looking at wind sock, and wait to be accounted for by your area coordinator. * Once head count is completed, remain in the area until released or other instructions are issued. Note: If you are out of your department, exit with the department you are in, notifying their area coordinator that you are from another department, what department you are from and who your area coordinator is. Note: If you are out of department, exit with the department you are in, then proceed to the Perishable parking area to be counted by your supervisor. AREA COORDINATORS: * After hearing the alarm or passing the order to evacuate to employees under your immediate supervision, exit the building using Perishable's entrance. * Once outside, proceed to the employ ee parking lot, assembling your department to take head count. Champaign Distribution Center Approved Policies and Procedures Approved By: Effective Date: Revision Date: 2/1/99 Subject: Emergency Action Plan 400 Ammonia Release, Explosion, Fire 404 FIRE/EXPLOSION/AMMONIA RELEASE The following activities should be performed when a fire or explosion occurs: a) The person(s) observing a fire or explosion should immediately report the situation to a member of management. b) Management will notify the fire department. c) Concurrently, the evacuation of personnel should be initiated via automatic alarm and P.A. d) The Emergency Team should monitor the evacuation of personnel. e) The Fire Department should be met by the supervisor in the affected area and directed to the emergency area. f) Employees should not attempt to fight fires unless they have been trained in fire fighting, including fire extinguisher training. At any one time, 60% of the ammonia is in the equipment room. The remainder is dispersed throughout the piping system located in the Perishable Warehouse. Ammonia is pumped from the equipment room to the Perishable Warehouse via schedule 80 pipe. Where feasible, piping has been incorporated into the super structure of the building or protected by wooden barriers to prevent accidental release due to striking by forktrucks or other equipment. Pressure relief valves venting to the roof have been installed in the system to avoid a release being dispersed in the building, and inspected annually. AMMONIA RELEASE PROCEDURES The following procedure has been developed to ensure the release of ammonia, accidental or otherwise, is promptly investigated for employee exposure and to expedite repair, if required. Employees play a key role in the avoidance of implementing this plan by being aware of the presence of ammonia piping and equipment in their work area. Should an employee strike or observe the striking of ammonia related equipment, supervision must be notified immediately to implement corrective action, minimizing employee exposure. Failure to report such incidents could result in a delayed response. Employees Role: 1) Employees discovering or observing the breakage of an ammonia line or detecting a strong ammonia odor should immediately report the incident to their supervisor. Annual review of new hire training on ammonia procedures. Supervisors Role: 1) Upon notification, the supervisor should contact both Maintenance (ext. 2784) and Risk Control (ext. 2719 or by pager at 1-888-976-1060) to investigate the situation. If warranted, based on assessment, the area should be evacuated. 2) If a major release is observed, the whole department should be evacuated. Note: If unable to reach Maintenance, the guard office (2888) should be contacted to notify the Maintenance person on call and advise them of the situation. Through development and implementation of this plan, SUPERVALU Champaign Distribution Center ensures compliance with OSHA 1910.120 Hazardous Waste Operations and Emergency Response, Sectio n Q. Facility Description SUPERVALU Champaign Distribution Center is a grocery distribution center, located at 2611 North Lincoln Avenue in Urbana, Illinois. The structure covers a total of 16.9 acres under one roof, sub-divided into four major segments; Dry Grocery Warehouse, Perishable Warehouse, Truck Servicing Garage and General Office areas. Of these areas, Dry Grocery accounts for 569,000 square feet of floor space. The main hazard associated with this area is the tremendous fire load due to the racking and stacking of palletized dry products in cardboard containers. The Perishable Warehouse, home of produce, dairy and frozen food, utilizes 168,200 square feet. This area is temperature controlled through use of Anhydrous Ammonia as a refrigerant. Through use of Anhydrous Ammonia, the department is able to control temperatures ranging from 55 degrees in the produce room to 20 degrees below zero in the freezer. The main hazard in this area is the potential release of Anhyd rous Ammonia to the atmosphere in liquid or gaseous form. The Garage area accommodates only 12,300 square feet. The Garage did service all SUPERVALU fleet vehicles, both auto and truck, and contained numerous oil and petroleum base products in quantities less than 60 gallons. This facility is now closed and the worked performed will be done off site. General Offices make up the remainder of the square footage. Located in the front of the main warehouses, the general offices contain the general fire load of any commercial property, i.e., paper, furniture, fixtures, etc. Located on the first floor, Northwest corner of the administrative offices, is the Printing Department. Within this department, small quantities (less than one gallon) of inks and solvents are used on a daily basis. Larger quantities of these chemicals in 55 gallon drums are stored in the Forward Buy section of the Dry Grocery Warehouse. Those liquids within the flammable range have been stored in flammable stor age cabinets. AMMONIA CHARACTERISTICS Common/Chemical Names: Ammonia, Anhydrous Ammonia, NH 3 Physical/Chemical Properties: - Colorless gas or liquid with strong pungent odor. - Strong caustic (base) material - Ph of 11.6 - Lighter than air - will rise to ceiling when released - Non-flammable in small concentrations. - Lower explosive limit - 16-25% in air. Health Effects: Inhalation: Ammonia gas is extremely irritating to the mouth, throat, and lungs. A sore throat, soreness of breath and labor breathing can occur after extended exposure. Skin & Eyes: Direct contact with liquid ammonia results in immediate redness and corrosion and possible frost bite to the area. First Aid Procedures: Inhalation: Immediately leave the area up wind of the release. If breathing has stopped, administer artificial respiration. Skin: Immediately flush the affected area with large amounts of water for at least 15 minutes. Clothing frozen to the skin should be thawed prior to removal.
